P4282 - Evaluating the Need and Impact of a Multifaceted Wellness Program on the Health-Related Quality of Life (HRQOL) in Female Patients With Inflammatory Bowel Disease

Introduction: Women with IBD have reported increased levels of stress and lower health-related quality of life compared to men with IBD. There is scarce data on interventions for this disparity. We aimed to design, implement and study a multifaceted wellness program for women with IBD that enhanced their existing medical management.
Methods: A total of 100 women diagnosed with IBD were enrolled in an observation (N=50) or intervention (N=50) group. Both arms received standard of care and surveys, but the intervention arm was also enrolled in a wellness program. The intervention consisted of two virtual sessions of four healthy living classes and seven individualized wellness coaching sessions. The first virtual session involved an IBD-specific nutrition class, then a virtual session including group classes on stress management, resiliency, physical activity, and healthy meal planning. Participants were sent digital surveys which consisted of demographics, lifestyle, IBD status, PROMIS 10 for Global Health, quality of life (LASA6), Perceived Stress Scale-4, Female Sexual Function Index-IBD, resiliency, burnout, and patient satisfaction. These were assessed at baseline, three months, six months, and twelve months.
Results: Mean age among participants was 40.8 ± 12.0 years. Other demographic characteristics including ethnicity, race, marital status and education were comparable between both arms (Table 1). The observation arm was more likely to have smoked in their lifetime (p=0.02) and less likely to have been diagnosed with depression in the past (p=0.01). At three months, Female Sexual Function Index-IBD was significantly lower in the intervention arm compared to the observation arm (p=0.04) (Figure 1). At six months, there was a significant improvement in quality of life (LASA6) for the patients in the intervention arm compared to the observation arm (p< 0.01). Significantly more women in the intervention arm felt the study was worth their time (p=0.04) and that the study experience was better than expected (p< 0.01).
Discussion: A multifaceted wellness program had a positive impact on quality of life, sexual function and patient satisfaction for women with IBD. This study demonstrates the feasibility and effectiveness of virtual delivery of healthy living classes and wellness coaching. These lifestyle interventions can enhance standard medical care in chronic disease management for women with IBD.
