Introduction: Background: IgG4-related sclerosing cholangitis is rare disease, presented as a biliary manifestation of IgG4-related disease. Although serum levels of IgG4 are important in diagnosis establishment, it is still unclear whether they correlate with the disease activity.
Aim: To investigate the correlation of serum IgG4 with laboratory parameters of cholangitis.
Methods: Levels of IgG4 from serum of 22 patients with established diagnosis of IgG4-related sclerosing cholangitis were analyzed by nephelometry. Patients were divided in two groups based on the levels of serum IgG4, bearing in mind referent values of 1.35g/L. Statistical analyses included descriptive statistics, Student’s t test, Spearman’s correlation.
Results: In patients with IgG4-related sclerosing cholangitis, 11 (50%) were female, with mean age of 40 years (ranging from 18 to 61). Mean concentration of serum IgG4 was 2.84 g/L. Levels in female patients were statistically significantly higher compared to male (F=5.041, P=0.049). Furthermore levels of alkaline phosphatase, as well as serum amylase and lipase were significantly higher in the group of patients with elevated IgG4 (P=0.04, P=0.05 and P=0.05) respectively. Moreover, elevated parameters of systemic inflammation were seen in the group of patients with elevated IgG4, ESR, CRP, neutrophil to lymphocyte ratio and platelet to lymphocyte ratio (P=0.03, P=0.05, P=0.04, and P=0.05).
Discussion: Conclusion: Our results show that IgG4 levels significantly correlate with activity markers of IgG4-related sclerosing cholangitis.
