Introduction: Metabolic Dysfunction-Associated Steatotic Liver Disease (MASLD) has been increasingly recognized as a significant health issue with potential systemic implications. Barrett’s esophagus (BE) is a premalignant condition associated with gastroesophageal reflux disease. Both MASLD and BE share similar risk factors such as obesity, male gender, older age, diabetes etc. However, its association with BE has not been well studied. Our study aims to assess the association between MASLD and BE.
Methods: We conducted a retrospective cohort study using the TriNetX multi-institutional database. We identified adults (age >18) with MASLD who had undergone EGD following their diagnosis. These patients were compared to individuals who had an EGD but never had MASLD. Propensity score matching was performed to adjust for age, sex, race, comorbidities, BMI, gastroesophageal reflux disease (GERD), Helicobacter pylori infection, and proton pump inhibitor use. A subgroup analysis was performed for obese patients (BMI > 30).
Results: There were a total of 2,357, 216 patients who underwent EGD of which 153,262 patients with MASLD and 2,203,954 patients without MASLD. After propensity score matching, each cohort comprised 152,328 patients. Patients with MASLD were found to have a higher prevalence of BE (8,677 [5.69%]) compared to those without MASLD (8,044 [5.28%]) (aOR 1.083, 95% CI 1.05-1.118, p < 0.0001). In a subgroup analysis of patients with obesity (BMI > 30), the risk of BE was also higher in obese patients with MASLD (5,602 [5.96%]) compared to obese patients without MASLD (5,082 [5.40%]) (aOR 1.109, 95% CI 1.066-1.153, p < 0.0001).
Discussion: Our study found that MASLD is associated with an increased prevalence of BE, independent of obesity. These results highlight the importance of screening for BE in patients with MASLD. Further research is needed to evaluate this association more thoroughly.
