Introduction: Disorders of the gallbladder are significant contributors to mortality among older adults. This study investigates trends and demographic disparities in mortality due to gallbladder diseases among patients aged 25 and older in the United States from 1999 to 2020.
Methods: Using data from the CDC WONDER database, a retrospective analysis was conducted. Age-adjusted mortality rates (AAMRs) per 100,000 persons were calculated, and trends were evaluated using Average Annual Percentage Change (AAPC) and Annual Percent Change (APC). Data were stratified by year, sex, race/ethnicity, and geographical regions.
Results: Between 1999 and 2020, gallbladder diseases accounted for 109,262 deaths among adults aged 65+ in the U.S. The majority of fatalities occurred in medical facilities (71.5%). The overall AAMR for gallbladder diseases-related deaths among older adults decreased from 14.6 in 1999 to 10.8 in 2020, with an AAPC of -1.61 (95% CI: -1.77 to -1.46, p < 0.000001). Notably, there was a significant decline in AAMR from 1999 to 2015 (APC: -2.28, p < 0.000001), followed by a slight increase from 2015 to 2020 (APC: 0.57, p = 0.240). Stratified by sex, older men exhibited higher AAMRs compared to women (men: 13.8; women: 10.5). Both genders experienced decreased AAMRs, with a more pronounced decrease in women (men: AAPC: -1.46, p < 0.000001; women: AAPC: -1.92, p < 0.000001). Racial disparities in AAMRs were evident, with Americans having the highest rate (17.2), followed by Hispanics (13.2), Whites (11.8), Blacks (11.3), and Asians (10.5). All groups saw significant decreases from 1999 to 2020, notably Black individuals (AAPC: -2.65, p < 0.000001). Geographically, AAMRs varied, with Vermont showing the highest (17.2) and Florida the lowest (7.9) mortality rates. The Western region had the highest AAMR (13.5). Nonmetropolitan areas (14.4) had slightly higher AAMRs than metropolitan areas (11.3) throughout the study period.
Discussion: The study notes a decline in gallbladder disease mortality rates among older adults in the US from 1999 to 2020. Despite this, demographic disparities persist, underscoring the importance of targeted interventions and equitable healthcare access. Further research is needed to understand the factors behind these disparities and develop effective public health strategies.
