Introduction: Women have been historically underrepresented in gastroenterology and disparities extend to advanced endoscopy due to multiple factors such as stringent hours, radiation exposure during fluoroscopy, and lack of female mentors. Through this study, we aimed to analyze and quantify the gender trends in first and senior authorship over the past 8 years in endoscopy journals.
Methods: We screened the top 3 endoscopy journals based on their impact factors as listed on their respective websites - Endoscopy, Gastrointestinal Endoscopy, and Digestive Endoscopy. The original articles, reviews, and meta-analyses published from 2016 to 2023 were selected. Data was collected on the gender, h-index, citations, position, and affiliation of first and senior authors. Gender was determined via a Google search of the author’s name and affiliation. H-index and citations were extracted from Google Scholar and Scopus. Data was analyzed using pivot tables and the Chi-square and Mann-Whitney U tests were performed using the online Social Science Statistics’ calculator.
Results: Among a total of 1323 articles, 1462 unique first and senior authors were identified. Gender was determined for 92% (N=1345) of authors, out of which 84.2% were males and 15.8% were females; p< 0.01. Women were the first authors in 20% of the articles, and senior authors in only 5.2%. Women represented 42% of full-time researchers, while only 14% of academic physicians compared to 86% of men; p< 0.01. From 2016 to 2023, the female first authorship increased from 16.7% to 21.7% and the senior authorship rose from 2% to 6.1% (Figure 1). The median (IQR) h-index for first-author females was 9 (5-16), which was significantly lower than that of first-author males, 16 (8-27); p< 0.01. Similarly, the h-index for female senior authors was significantly lower than that of males (29.5 vs 35); p< 0.01 (Table 1).
Discussion: Although the number of female authors was significantly lower than that of males, their contribution has increased over the past 8 years, especially amongst senior authors, albeit with a greater gender gap. Both the first and senior author h-index was significantly lower for women than men, highlighting the need for continued efforts to promote gender equality in authorship and support female researchers in their career advancement. This study highlights the discrepancies in leadership within academia with the recommendation to strategize new policies for gender equity in the research field.
