Introduction: Patients with obesity who have more frequent contact with the medical system generally have improved outcomes. Rates of post-procedural follow-up after endoscopic sleeve gastroplasty (ESG) vary widely, which may have palpable implications for the degree of post-procedural weight loss patients can achieve. This study sought to determine the relationship between post-ESG follow-up visit compliance and total body weight loss within one year post-ESG, among patients who received either ESG alone or ESG combined with sequential pharmacotherapy.
Methods: A single-center retrospective study was performed for all patients who either underwent ESG or combined ESG with pharmacotherapy at a large academic tertiary care center from June 2021 to September 2023. Compliance with follow-up visits, assessed as the total number of nutrition and/or gastroenterology visits per year, was recorded. Patients were split into 3 groups: lowest, moderate, and highest compliance. Percent total body weight loss (% TBWL) at 1, 3, 6, and 12 months post-ESG was collected. % TBWL was defined as weight loss divided by pre-ESG weight. Demographic data, including age, gender, pre-ESG weight, and pre-ESG BMI, were also collected.
Results: Demographic data (lowest, moderate, highest compliance) included number of subjects (44, 48, 44), average age in years (42, 44, 46), pre-ESG weight in pounds (222, 217, 208), and pre-ESG BMI (36, 37, 36). Notably, although there was no significant difference in % TBWL 1 month post-ESG between the 3 groups, both the moderate and highest compliance group experienced significantly higher %TBWL than the lowest compliance group at 3 months and 12 months. At 6 months, there was also a significant difference in %TBWL between the lowest and highest compliance groups. There was no significant difference in % TBWL at any time between the moderate and highest compliance groups (Table 1).
Discussion: Our findings suggest that increased follow-up visit compliance is a key component of continued weight loss in patients who undergo ESG. It is possible that closer follow-up is a motivator for patients to continue lifestyle and pharmacotherapy interventions; conversely, patients who tend to be more compliant with such interventions may tend to attend more follow-up visits. While the exact causative relationship merits further exploration, our study emphasizes the importance of close post-procedural follow-up in maintaining durable weight loss outcomes for patients with obesity.
