Introduction: With the increase in number of liver transplant recipients over the last decade, there are limited data available about the nationwide trends of complications leading to readmissions post-Endoscopic retrograde cholangiopancreatography (ERCP) in these patients.
Methods: Patients with a history of liver transplant undergoing ERCP from January 2018 to November 2018 were identified using International Classification of Diseases, 10, Clinical Modification (ICD-10-CM) codes from the healthcare cost and utilization project’s (HCUP) national readmission database (NRD). The common causes of unplanned 30-day readmission after ERCP procedure were determined. Multivariate logistic regression analysis was used to analyze the predictors of readmission. Discharge weights were applied to provide national estimates.
Results: During the study period in 2018, 2205 ERCPs were performed in liver transplant recipients. Of them, 200 (9.07%) had at least one unplanned readmission within 30 days. Sepsis (12.50%), enterocolitis due to C. difficile (5.39%), acute kidney failure (4.69%), angiodysplasia of stomach and duodenum with bleeding (4.11%), and anemia (3.14%) were the top 5 causes of readmissions in 30 days and accounted for almost third of all readmissions. Significant predictors of readmissions included nonalcoholic hepatic steatosis (OR 3.56; 95% CI 1.66-7.64; p=0.001) and metastatic cancer (OR 4.89; 95% CI 1.55-15.48; p=0.007).
Discussion: Unplanned readmissions in liver transplant patients undergoing ERCP remain significantly high, especially in the first 30 days post-procedure. Our study highlights the predictors of readmission and the unmet need to improve the adverse event profile in these patients.
