Introduction: The use of Glucagon-like peptide-1 (GLP-1) agonists for diabetes and obesity has been increasing over the past few years. These drugs mimic the action of the naturally occurring hormone GLP-1. In addition to glycemic control through glucose metabolism, it can affect appetite through slow gastric emptying. Often retained gastric contents on endoscopy in patients on GLP-1 agonists can lead to aborted procedures. We aimed to find evidence on GLP-1 agonists causing a delay in gastric emptying through scintigraphy imaging.
Methods: We searched Pubmed, Embase, and Cochrane from inception to May 2024. Search words included ("GLP1 agonists" OR "GLP-1 analog" OR "liraglutide" OR "semaglutide" OR "exenatide” OR “lixisenatide” OR “albiglutide” OR “dulaglutide” OR “Tirzepatide”) AND ("Gastric emptying" OR “residual gastric food” OR “residual gastric content*”). Data on patient characteristics, GLP-1 agonist, and Scintigraphy imaging results were collected. Descriptive statistics were employed, with means and standard deviations for continuous variables and percentages for dichotomous and categorical variables.
Results: We identified 10 studies on scintigraphy imaging in patients on GLP-1 agonists.(Table-1) There were 239 patients 144 patients on GLP-1 and 141 patients on placebo. The medications used were Exenatide (5), liraglutide (2), semaglutide (1), ROSE-010 (1), GLP-1 (7–36)amide (1). The commonest indication for usage was obesity, Type 2 DM. In all the studies GLP-1 agonists significantly affected gastric emptying p< 0.05 irrespective of the duration of drug use. Discussion: GLP-1 agonists can delay gastric emptying irrespective of duration of use. Further studies on dosing, longer duration of use, tachyphylaxis, and fasting guidelines before endoscopy procedures can help decrease adverse events and procedure cancellations from retained gastric contents.
