Introduction: The green kiwifruit (Actinidia deliciosa, cultivar Hayward) and the gold kiwifruit (Actinidia chinensis, cultivar Zesy002) have been extensively studied for their positive effects on physical and mental health. Several clinical intervention trials have investigated the impact of these kiwifruits on constipation, using the Bristol Stool Scale (BSS) to evaluate stool type. Our objective was to review the evidence from clinical trials, concentrating on the effects of kiwifruit on gastrointestinal symptoms in both healthy individuals and those with functional constipation, including irritable bowel syndrome with constipation (IBS-C).
Methods: Electronic databases were searched from inception to May 2024. We included clinical trials examining the gastrointestinal (GI) effects of green or gold kiwifruit, or their compound in healthy participants or those with functional constipation (FC) plus irritable bowel syndrome with constipation (IBS-C). Mean difference (MD) with confidence intervals (CI) were determined for the following outcomes: frequency of complete bowel movements CBM as well as stool type as determined by the Bristol Stool Scale (BSS) for both the healthy individuals and those with functional constipation plus irritable bowel syndrome type constipation (IBS-C).
Results: Eight randomized controlled trials (RCTs) with a total of 662 participants were included in the analysis. When compared with a placebo group (n = 569), kiwifruit extracts did not significantly affect the weekly frequency of complete bowel movements (CBM) in healthy populations (mean difference [MD]: 0.18; 95% confidence interval [CI]: 0.01, 0.36) and has no effect in patients with functional constipation plus irritable bowel syndrome type constipation (IBS-C) (MD: 0.86; 95% CI: -0.18, 1.90). Regarding the Bristol stool scale, no significant mean difference was observed between kiwifruit and placebo in healthy participants (MD: 0.62; 95% CI: -0.28, 1.52) or in those with functional constipation plus irritable bowel syndrome type constipation (IBS-C) (MD: 0.35; 95% CI: -0.03, 0.72)
Discussion: Among individuals with constipation, there is no evidence that kiwifruit may increase complete bowel movements (CBM) and Bristol stool score when compared to placebo or psyllium.
