Introduction: Foreign body can cause myriads of intestinal complications such obstruction and perforation. There have been over three hundred cases of bowel perforation that have been reported in literature with fish bones, chicken bones and dentures. We present a case of chicken wing causing contained perforation of the colon which was found during colonoscopy.
Case Description/Methods: A 43-year-old male patient underwent diagnostic colonoscopy for concerns for alteration in bowel habits and intermittent hematochezia. The symptoms had started 6 months back with passing of stool 6-7 times with minimal stool passage. There was significant weight loss of 20 lb over the same period. No abdominal pain, Nausea, Vomiting. no F/h malignancy or IBD.
During colonoscopy, sigmoid colon was found to have inflamed mucosa, edema with stricturing in the sigmoid colon. There was also a bone piece visible which could only be removed in pieces with the help of snare. CT scan of pelvis was done which showed contained perforation with foreign body in situ. The patient had to undergo non urgent sigmoid colon resection. Currently the patient is doing well and has recovered fully.
Discussion: Most of the foreign bodies pass through the intestine without any complications. The type of complications depends on the size, shape and material of the foreign body. Foreign body getting impacted in the sigmoid colon is rare. The area most commonly affected is the narrowest part of the intestine near the terminal ileum. Symptoms can often be delayed. Most common symptom is nonspecific abdominal pain. X-ray and CT scan can be used to help in diagnosis. Simple impacted bone can be removed with the help of endoscopy but complicated cases such as obstruction, abscess, fistula need surgical resection.
