Introduction: One barrier to CRC screening among Hispanic adults may be language as about 30% of the U.S. Hispanic population are fluent only in Spanish. The multi-target stool DNA test (mt-sDNA) is a guideline-recommended, at-home CRC screening option for average-risk individuals. The test includes access to navigation support around-the-clock for both patients and providers, with translation services for over 240 languages, including Spanish. There is limited evidence on longitudinal screening data for mt-sDNA test in Spanish-speaking patients in the U.S. This study aims to evaluate the adherence rate of patients previously screened with mt-sDNA test across payors in patients selecting Spanish as their preferred language.
Methods: Patients aged between 45 and 85 years, who had previously completed a valid mt-sDNA test with negative result ≥ 2.5 years prior, were shipped a mt-sDNA test kit by Exact Sciences Laboratories between January 1, 2023, and December 31, 2023, with an order for subsequent testing by the patient's provider. The patient adherence to mt-sDNA is defined as completion, return of the test kit and a valid result within 120 days from the initial shipment.
Results: Of 9,446 eligible study participants who reported that Spanish was their preferred language, the overall adherence rate for subsequent testing was 76.7% with mean (SD) time to test return of 20.2 (16.5) days. For age groups 45-64, 65-75, and 75-85 years, adherence rates were 78.0%, 76.1% and 70.8%, respectively. There was a statistically significant difference in the overall adherence among the age groups (p< 0.001) and different patient outreach channels (p< 0.001). Adherence rates for subsequent testing were as follows for various payors: Managed Care Organization at 79.7%, Medicare Advantage at 77.6%, Commercial at 76.2%, Medicaid at 69.0% and Medicare at 74.8%.
Discussion: This study demonstrates that personalized patient navigation outreach with Spanish language in Spanish-language preferred population resulted in adherence rates exceeding 70% for CRC rescreening, overall and in all analyzed subgroups. The study also highlights the importance of subsequent testing that is essential for the effectiveness of CRC screening to identify cancers manifesting over time in an individual and do so in the early stages where it is more easily treated.
