Introduction: Congestive Hepatopathy is the manifestation of chronic congestion of the liver secondary to heart failure or other cardiac defects that ultimately lead to an increase in central venous pressure. Limited data is available on the impact of hyperlipidemia on congestive hepatopathy. We sought to examine the national inpatient sample database to describe in-hospital outcomes among these patients.
Methods: Data were extracted from the National Inpatient Sample (NIS) Database for the years 2019 and 2020. The NIS was searched for hospitalizations of adult patients with congestive hepatopathy. We then examined the outcomes of patients who also had hyperlipidemia. Multivariate logistic was used to adjust for confounders. The primary outcome was inpatient mortality. SPSS software was used for statistical analysis.
Results: This study included 17,917 patients with congestive hepatopathy, of which 8082 (45.1%) also had hyperlipidemia. Patients with hyperlipidemia had a higher prevalence of HTN (87.5% Vs 76.6%, p< 0.001), CKD (52.1% Vs 46.4 %, p< 0.001), obesity (13.72% VS 11.9%, p < 0.001), and smoking (27.2% Vs 23.7%, p< 0.001). In-hospital mortality was lower among patients with hyperlipidemia (6.7% Vs 7.3% p< 0.001). Multivariate regression showed that hyperlipidemia patients with congestive hepatopathy had higher inpatient mortality (OR 1.715, Cl 1.638-1.795, p< 0.001). On secondary analysis, it has shown that older patients with congestive hepatopathy had higher odds of having NSTEMI (OR 2.387, CI 2.206-2.582, p< 0.001), pressor requirements (OR 1.581, CI 1.502-1.665, p< 0.001), CKD (OR 2.028, Cl 1.984-2.072, p< 0.001) and arrhythmias (OR 1.931, CI 1.884-1.980, p< 0.001).
Discussion: In this nationally representative populationābased retrospective cohort study, patients with hyperlipidemia were associated with worse outcomes among patients with congestive hepatopathy.
