Introduction: Endobariatrics is an emerging field that allows gastroenterologists to perform minimally invasive bariatric procedures including endoscopic sleeve gastroplasty (ESG), intragastric balloon (IGB), and bariatric surgery revisions. Many patients continue to face struggle with weight loss and endobariatric procedures hold significant potential as a weight-loss alternative.Due to this field's novelty, there are many steps to account for in establishing an endoscopic bariatric practice, specifically within a large private practice model.
Methods: Setting up an endobariatric practice involves several key steps. First, gaining familiarity through shadowing and training is essential. It is advised to attain an obesity board certification and attend courses to become proficient in endosuturing. Next, all members of the private practice need to be informed of the financial and marketing aspects. Negotiating the logistics of these procedures, coordinating the aftercare (hospital or ASC), and assembling a nutrition team is crucial. Lastly, establishing a dedicated website, active social media presence, and conducting grand rounds in regional hospitals can raise awareness about endobariatrics.
Results: The major steps detailed above were followed in order to establish an endobariatric center under a large private practice model. Three physicians in the largest private practice group in upstate NY interested in endobariatrics individually received the appropriate training and certification required based on recommended guidelines. Other members of the private practice were introduced to and approved the establishment of an endobariatrics program. The inclusion of a nutrition team has been important in facilitating lifestyle changes and maintaining weight loss post-procedure. Furthermore, a digital presence (e.g. TikTok and Instagram) has been a quick reference for patients seeking information on procedures and associated costs.
Discussion: These steps have worked to successfully set up an endobariatrics center under a larger private practice model.Given endobariatrics is an emerging fieldwith few established clinics, other important aspects include the location of the clinic, the size of the team, and ensuring social media presence reaches target populations. Additionally, addressing current cost barriers with insurance coverage is an important future consideration.
